My name is Annie Faulkner. I am the lead social prescriber for EEAST and am working alongside Norfolk and Waveney Integrated Care Board (ICB) to deliver social prescribing and one-off personal health budgets (OOPHBs) within Norfolk and Waveney at EEAST.

We offer a person-centred conversation based around asking ‘what matters to you’? This is with the aim of working with people in the local area to connect them to community-based services, such as groups and activities, to meet their practical, social and emotional needs which affect their health and well-being. 

We also use personalised care and support planning conversations with the patient (and/or carers) to deliver one-off personal health budgets (OOPHBs). The goal is to reduce avoidable conveyances / unnecessary admissions.

We are looking for Champions to promote social prescribing and one-off personal health budgets within EEAST by referring them to our team to enable support for admission avoidance. We are recruiting Champions in the Waveney area initially, before rolling this out to other areas within the Norfolk and Waveney locality.

This is a 6-month pilot project running until December 31st 2023. 

Referrals from EEAST can be made 24/7 via email link or by scanning a QR code – all you need to do is complete a short form and submit.

As the lead social prescriber for EEAST, I will be working alongside the ICB social prescribers who are based at the 3 acute hospitals in the region - James Paget, Norfolk and Norwich and Queen Elizabeth. The Teams work Monday- Friday 09:00-17:00 and will pick up referrals during these hours.
