Have you had your flu jab elsewhere?

Our 2022 Flu Campaign is underway with many of you being vaccinated already. We are aware that not everyone would like the vaccine or has received a flu jab elsewhere such as at your own GP surgery.

If you have the vaccine at a location other than within the Trust it is important that you let us know you have been vaccinated so we can update our own records. We are required to report that all staff have been offered the vaccine.

• Please complete the form detailing that you have already been vaccinated elsewhere.

Thank you to everyone who has either had a vaccine so far or completed the appropriate forms to say you have declined or had it elsewhere, your participation is greatly appreciated.

If you have not had your vaccination but still wish to do so please visit our flu clinic page. If there is not a clinic available to you locally please email flu@eastamb.nhs.uk and they will be able to assist you.
