During the hours of 0800-2000 please ‘Call Before You Convey’. The patient must not be time critical, have no red flags and not destined for a pre-defined pathway such as stroke, PPCI, trauma etc. All discharge conversations and plans must be in line with the EEAST Safe non-conveyance discharge policy and carefully documented. Please have the patient’s details including NHS number available.
