New Interim Cohorting Data Capture Process

Following agreement from OSDG-formal on 7th Dec 2021, a process to capture cohorting data will go live across EEAST on the 13^th Dec 2021.

• Link and QR Code for MS FORM for inbound crews to use to submit data when handing over patient to a EEAST Cohorting Team. – Please make sure all data is cleared when entering the next CAD data.
• Link and QR Code for MS SHAREPOINT which will display the MS forms entry submitted by the In-bound crew and allow the Cohorting Team to enter date and time of Patient Handover to Hospital.
