Pathway change for conveyance of OHCA patients in Essex

Clinical Instruction

From 17th October 2022, there will be a change in the inclusion criteria for people requiring PPCI where Basildon is the closest Hospital with PPCI speciality. This change will only be for patients going to Basildon, all other PPCI centres will not be adopting this change at present.

The ambition for the algorithm change is to deliver the correct cardiac arrest patient to the right treatment centre, with minimal secondary transfers, and simplify the triage decision making for pre-hospital clinicians

The Essex Cardiothoracic Centre (ECAC) are requesting that out-of-hospital cardiac arrest patients who have who have ROSC and a presenting rhythm of VT/VF or the presence of an ST elevation myocardial infarction (STEMI) on a post ROSC ECG are rapidly transported with a pre-alert directly to the ECAC.

For patients with an initial non-shockable rhythm an no evidence of STEMI, the pathway remains the same and pre-hospital care providers are requested transport them to the nearest DGH ED.

The ECAC will be closely monitoring this change through our cardiac arrest registry, alongside data inputted regionally daily and shared with EEAST via a data sharing agreement. Please review the short video link below which explains the pathway. If you have any questions about this pathway, please email carl.smith@eastamb.nhs.uk
