More defective adrenaline found

A further batch of defective adrenaline has been found.

Following our clinical update of 16th April which alerted staff to potentially defective batches of adrenaline, another suspect batch has been discovered.

We have had a report of an empty Aurum Adrenaline 1:10,000 syringe.

The unit was sealed just before opening, when opened it was discovered the syringe was empty with plunger depressed.

The batch number was 0098831.

This was similar to an issue from April 2019 where some were found with the plunger drawn back and some with the plunger depressed.  There was no evidence of leakage from the syringe.

Please check drug bags and stores. Should you find evidence of this please let the Tactical Operations Centre know as soon as possible by emailing Operations@eastamb.nhs.uk.
