Covid Grab and Go Guide

The Grab and Go guide has been designed in partnership with people with learning disabilities, their families and medical staff.

The guide can help provide information that doctors and nurses will need if patients go to hospital because of COVID-19 and, for example, are struggling to breathe.

Please note, it is not a replacement for the everyday, detailed hospital passport.

Patients are instructed to update their hospital passport where needed. Please ensure that the hospital passport and/or Grab and Go guide travel with the patient. This is especially important if the patient is travelling alone.
