Equipment fails and near-misses

Had an equipment failure or near-miss? Please remember to report it on Datix.

The clinical engineering team use the information you submit to put problems right and reduce the risk of any reoccurrences. The team monitors reports closely, and there are currently some discrepancies between localities in terms of the numbers of reports they see.
