Hospital reminder about patient’s medication

A hospital in our region has found that not enough patients are bringing their medication when taken to hospital by Trust staff.

It is essential that patients bring their medication to hospital so that doctors and nurses can get an accurate picture of a patient’s current drug history and to avoid missing any doses. Wards and pharmacies do not carry every medicine as stock and this best practice avoids confusion and wastage.

Please remember to use the Trust-issued green bags to take all of a patient’s medication when you convey them to hospital.

There are some areas where specific guidance may be issued around prepared multi medication dosage packets.  If you have any questions, please speak to your duty locality officer team.
