Launch of the new Patient Care Record Policy

The Trust have reviewed, amended and updated the PCR policy.

Please make sure you are familiar with the policy and its requirements to make sure that a thorough PCR is completed upon every incident, especially where face to face contact has been made.

PCRs are classed as medico-legal documents which can and will be used at Coroner or Crown Court.

Registered healthcare professionals need to be aware that this is a fundamental element of the HCPC’s standards of proficiency.

Remember if it’s not documented, it didn’t happen. Electronic Patient Care Records are the preferred method of completion and it is expected that if one is available it is to be used.  The Policy can be found here
