Leaver checklist: what to do when a member of staff leaves

When a member of staff leaves the Trust, their line manager has a responsibility to make sure all relevant checks are completed and the relevant documents sent to HR. 

A HR3 form, which can be downloaded from East24, needs to be filled out and returned to HR when a member of staff leaves the service. The form covers when the person is leaving, why, and checks that all equipment has been returned etc. 

In addition, line managers must make sure that: 

  • an exit interview is completed with the member of staff before they leave
  • all Trust property has been returned, including ID badges, door access fobs, uniform, IT equipment, clinical equipment and drugs
  • the IT team has been informed so IT accounts can be shut down (itservicedesk@eastamb.nhs.uk).
  • payroll has been informed.
