New diabetic referral pathway in Norfolk

There is a new referral pathway for patients who have had a diabetic hypoglycaemic event in Norfolk, Great Yarmouth and Waveney.

A referral should be made to SPOC and an information leaflet given to all patients, so that an external diabetic education team can follow up with them to offer support to help better manage their condition.

Consent to refer the patient is not needed at this point, as there is a number to call to opt-out in the leaflet.

Leaflets will be on every vehicle in the area, so please remember to think hypo, think referral.
