What is an SI?

When things go wrong it’s important we understand why, and how we can limit the chance of it happening again. We do this all the time in our daily lives. At work, we sometimes need to more formally look at what’s happened so we as individuals and the Trust can learn.

This can look like a debrief (hot or cold) or a more formal investigation. For incidents that have, or could have caused patient harm, we have the serious incident (SI) process.

There’s lots of misinformation, horror stories and general un truths about the SI process. So we want to talk you through what an SI is, what it’s not, and how people are supported through them. There is a lot to talk about here, so please bear with us.
