Alternative Care Pathway for Peterborough

Working in partnership with the Greater Peterborough Network (GPN) of surgeries in the Greater Peterborough area (PE1-PE7), the GPN have established  a single point of contact named the GPN Care Co-ordination Hub (CCH) specifically for EEAST clinicians.

The aim of the Care Co-ordination Hub is to reduce time on scene by enabling us to leave patients safely at home for rapid follow up, assessment with observations and, where necessary, fast track input from primary, social care, voluntary sector etc.

Additional HCA capacity will be provided by GPN to support patients to stay at home safely. This includes up to 5 days observation and additional support after significant intervention.

The HCA’s are skilled in:

  • Phlebotomy
  • BP
  • Pulse Ox
  • Heart Rate
  • Urinalysis

Patient safety will remain the highest priority. Calls to the CCH will be taken by the lead clinician facilitating a peer-to-peer joint clinical decision about the patients care. An initial HCA visit will take place within two hours of the referral being made.

The referral will be via a telephone line specifically made available for EEAST. Should the clinician be unavailable, the call will be answered by a call handler and the call passed to the clinician who will prioritise calling back. A protracted wait for call back is not anticipated. The case details will be recorded on the GPNs clinical records system Systm1. Our CAD number will be shared for reference and to support evaluation.

  • Exacerbation of COPD
  • UTI
  • Chest infection
  • Upper respiratory tract infection
  • Ear infection
  • Vertigo
  • “Off Legs”
  • Minor ailments
  • Prescriptions – no need to contact the patients GP

 Helen Lord, Operations Manager for the GPN says:

“GPN are excited to have this opportunity to work in partnership with EEAST. This pilot will support the patients of Greater Peterborough to receive support and coordination of care within the community. Patients will benefit from having home visit reviews, and comfort calls for the following days after being assessed.  The ambulance crew will be able to have confidence that a patient’s care will be delivered in the community including social care arrangements and issuing of needed medication.”
