The CAD markers team have been reviewing their processes for managing CAD markers. Currently, the team receive keysafe information in a number of formats and have sought to streamline the process.
They now have a dedicated webform that can be used by patients and family members to securely submit keysafe information to be stored on the CAD system.
If you are given keysafe information by either a patient or family member, please prompt them to visit the Trust website and click on contact us then Keysafe Access Form. The link to the form can be found here.
