New Tranexamic acid patient group directive

Clinical Instruction

The Trust has reviewed and approved an updated TXA PGD and this is now being rolled out effective from the 1st January 2022.

Key changes:

Additional indications:

  • Patients aged over 18 with a head injury and a GCS of less than 12
  • Haemorrhage due to disorders of obstetric origin e.g. antepartum haemorrhage, suspected placental abruption

Dose

  • No change in doses but paediatric does can (if required) be made up to 10ml with normal saline to aid slow administration. Previously this was 20ml.

Route:

  • Intramuscular administration is not permitted, this is despite JRCALC suggesting it may be administered via this route. EEAST is unable currently approve this route of administration.

All Paramedics and eligible Nurses will need to review and sign to confirm their understanding prior to using this medication for changes above before 28th February 2022. Once signed you may immediately follow the PGD.

To do this please review the PGD (in the documents below) and sign the signature page then pass to your line manager to also sign. You should retain a copy of the PGD for your reference. The PGD is also available on the Trust clinical app and on EAST24. Please note the acknowledgement function in the app is not being used to confirm understanding and you must physically sign a copy as well as acknowledging it within the app.

Guidance for managers:

Once you and a member of staff have signed the signature page, please scan this, and save the file as PDF with file name Surname_Staff ID e.g. Smith_123456678. It is important to save the file like this for governance purposes. If the file is sent without this file name format, it will be returned, and the staff member will no show as compliant.

Please keep a record locally of which of your staff members have signed so you can engage those who do not sign immediately.
