New Unmet Need Facilitator role

An exciting job opportunity has arisen as part of a pilot project to better support our patients in the community who are experiencing an unmet need. 

The Unmet Need Facilitator will contact patients to signpost them to a service that can better support their unmet need following a referral from one of our crews. 

The  primary role is to action the relevant referrals from front line clinicians post patient contact (face to face or remote). This will be undertaken via telephone to increase the accessibility and access to all patient groups. The facilitator will work with the referred person to identify appropriate external services and partners that may be able to assist in meeting the unmet need identified. The post holder will use the appropriate directories of services both internal and external to ensure that all support that can be offered is identified.

Detailed job description and main responsibilities

  • To manage and be responsible for the day-to-day response to referral through the Unmet Needs Navigator portal.
  •  In consultation with the MiDoS lead, be responsible for the provision of a quality service and ensure the prompt professional delivery of that service.
  •  To promote and enhance the image of the Trust at all times, in accordance with Trust Policy and Procedures, promoting good relations with the public, patients and other health and social care professionals through effective communication skills.

GradeNHS AfC: Band 4
Contract: Secondment: 9 months
Hours: Full time - 37.5 hours per week
