Patient care records for cohorted patients

The Medical Records team would like to thank you all for the amazing care you all provide to our patients and take the opportunity to remind everyone of the importance of accurate and thorough documentation.

We have noticed an increase of cohorted patients, whose patient care record documentation has been lacking essential information. PCRs have been sent in with no date and no incident number - without these it is very hard to link these back to the original PCR and then to be found on the system (if a paper PCR).

Records should not be identified by sticking a patient’s name/or sticker on the record.

Please remember that these are documents which could be requested by the patient, next of kin, GP, Coroner etc. and need to be completed correctly.

As a minimum, documentation for cohorted patients should include:

  • Date,
  • CAD number
  • Crew and patient details
  • Clearly state that this is a cohorted patient.
