The Disability Support Network (DSN) and the EDI team would like to know if you have ever requested any reasonable adjustments while working at EEAST. This is so we gain a better understanding of our staff and develop a better future for our employees.

Please note, this survey is for Disability Support Network members and other staff who have has reasonable adjustments.

The survey covers questions such as, have reasonable adjustments been approved and the type of equipment you received in way of adjustment.

The information you provide will be dealt with in strictest confidence and no one will be identified. However, if we find any information you provide to be abusive, we will have no option to highlight this to our Executive sponsor and be dealt with accordingly.
