A questionnaire looking into the health and wellbeing of anyone who works nights is available on East24

The Working Time Regulations 1998 requires the Trust to provide night workers with the opportunity of a free health assessment.

The regulations classify a night worker as someone who normally works at least three hours at night, between the hours of 11pm and 6am. All members of staff, including bank workers, who work these times can complete the form.

The questionnaire aims to make sure each person is fit and able to do night work, or to identify if they have any health problems that might affect or be affected by night work.

Completing the form is voluntary, but if for any reason you do not wish to complete it, simply sign the declaration and return the form to HR Services.

The questionnaire is completely confidential and will only be seen by occupational health, who will keep a copy on individual health records.

Please return any completed questionnaires in a sealed envelope to PAM c/o HR Services, Hammond Road, Bedford, MK41 0RG or simply scan and return to MK.Clinic@people-am.com.
