Further hydrocortisone injection recall

Following the update earlier this month, we’ve been notified of another potentially affected batch of hydrocortisone sodium phosphate injections, 100mg/1ml

Under guidance from the Norfolk and Norfolk Hospital, duty locality teams in Norfolk have been asked to recall the following batch due to abnormal discolouration: 

Batch number (on box): 041773

Expiry: 03/18

Lot code (on ampoule): 041 

Although this is a local recall rather than from the MHRA and the discolouration is not a risk to patients, colleagues are asked please to check stocks on station and drug bags; if you find any of the above batch and it appears discoloured, please remove and restock your supply. At this stage all other batch numbers, even those that have a similar yellowish colour, are approved for use.

Duty locality officers: please return any affected batches to your supplier, i.e. if you stock via Dunstable stores please return there, but if stocked from a hospital please return there.
