In May 2021, a Field Safety Notice (FSN) instructed the Trust to remove potentially faulty safety cannulas from use. The Trust had no choice but to find an alternative safety cannula within hours of receiving the FSN.
The decision was made to replace the cannulas with the widely used Polysafety-Adva product with the early flashback feature. This task was achieved across the Trust in under five days. We also had the suppliers visiting all of our eighteen hub stations in order to offer product and clinical support. You can view the cannula troubleshooting guide here.
Following various conversations with staff, including this item being discussed on our Thursday Executive Q&A sessions, it has become apparent that a proportion of clinicians prefer the previously used Becton Dickinson Venflon Pro Safety cannula.
